    Position Summary: 

    The Licensed Medical Social Worker is responsible for consultation and direction of Social Service in Home Care and Hospice. Provides orientation and education to the social service and nursing staff. Develops, organizes, and implements programs, policies and procedures, conducts case work services and counseling, and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates in CQI activities.

     

    Requirements:

    • Master’s Degree from a school of Social Work accredited by the Council on Social Work Education. 
    • One (1) or more years of previous experience in all aspects of social services and discharge planning.
    • A valid driver’s license, reliable automobile, and current auto insurance.
    • Maintains patient confidentiality at all times.
    • Attends annual review and in services.
    • Attends at least 1 staff meeting on a monthly basis.

    Responsibilities:

    • Assesses the social, spiritual, and emotional needs of the patient and family in order to estimate the patient’s and family’s ability to cope with the problems of daily living and a terminal illness.
    • Supervise, assess, and complete Initial and Annual Competency.
    • Assists the physician and other Hospice team members to understand the spiritual, emotional, and social factors related to the patient’s health problems so that each patient’s plan of care foster's spiritual well being and human dignity.
    • Assures that social service/counseling activities are integrated and coordinated within Hospice.
    • Develops specific, measurable, realistic plans and objectives, which enhance social service delivery.
    • Ability to perform a psychosocial assessment and reassessment on all terminally ill patients. This includes pediatric, adolescent, geriatric, and the general patient population.
    • Answers questions and offers emotional support to patient and family.
    • Implements the Plan of Care provisions to meet patient and family needs, which include social service goals for alleviating problems, supportive counseling, problem solving, community referrals, pre-bereavement, and bereavement care.
    • Regularly re-evaluates the patient’s and family’s psychosocial needs.
    • Preform other duties as assigned.
