Job Summary:  The Hospice Licensed Medical Social Worker is responsible for consultation and direction of Social Service in Hospice. Provides orientation and education to the social service and nursing staff. Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates in CQI activities.


The Hospice Licensed Medical Social Worker demonstrates Competency in the Following Areas:
Assesses the social, spiritual and emotional needs of the patient and family in order to estimate the patient’s and family’s ability to cope with the problems of daily living and a terminal illness.
Supervise, assess and complete Initial and Annual Competency on BSW. 
Assists the physician and other Hospice team members to understand the spiritual, emotional and social factors related to the patient’s health problems so that each patient’s plan of care foster's spiritual well being and human dignity.
Assures that social service/counseling activities are integrated and coordinated within Hospice.
Develops specific, measurable, realistic plans and objectives, which enhance social service delivery.
Ability to perform a psychosocial assessment and reassessment on all terminally ill patients. This includes pediatric, adolescent, geriatric and the general patient population.
Answers questions and offers emotional support to patient and family.
Implements the Plan of Care provisions to meet patient and family needs, which include social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
Regularly re-evaluates the patient’s and family’s psychosocial needs.
Demonstrates knowledge of clinical social work.
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial needs of patients and family.
Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
Communicates appropriately and clearly to physicians, staff, Clinical Nurse Manager and administrative team.
Coordinates and directs Social Service to ensure patients’ needs are met and hospice policy is followed.
Understands Hospice philosophy and issues of death and dying.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality.
Manages and operates equipment safely and correctly.
Demonstrates an ability to be flexible, organized and function under stressful situations.
The Hospice Licensed Medical Social Worker maintains a good working relationship both within the department and with other departments.
Consults other departments as appropriate to collaborate in patient care and performance improvement activities.
Maintains performance improvement activities for department.
Documentation meets current standards and policies.
Provides education to staff on performance improvement. Ability to plan and organize orientation and inservice training for staff members. Participates in guidance and educational programs.
Performs consultative activities including recommendations for changes in service delivery.


Professional Requirements:
The Hospice Licensed Medical Social Worker adheres to dress code, appearance is neat and clean.
Completes annual educational requirements.
Maintains regulatory requirements.
Maintains patient confidentiality at all times.
Reports to work on time and as scheduled, completes work within designated time.
Wears identification while on duty, uses computerized punch time system correctly.
Attends committee, CQI and management meetings as appropriate.
Attends annual review and inservices..
Attends at least 1 staff meetings, reads and returns all monthly staff meeting minutes.
Complies with organizational policies regarding ethical business practices.
Represents the organization in a positive and professional manner in the community.
Communicates the mission, ethics and goals of the organization.


Regulatory Requirements:
● Bachelor degree from school accredited by the Council on Social Work Education Current State Medical Social Work Licensure (state-specific).
● Clinically licensed as a LCSW or ASW by the Board of Behavioral Science Examiners preferred.
● One more years of previous experience in all aspects of social services and discharge planning.
● A valid driver’s license, reliable auto, and current auto insurance.


Language Skills:
● Able to communicate effectively in English, both verbally and in writing.
● Strong written and verbal skills.


Skills: Basic computer knowledge.
