Position Summary:
The Home Health Care Medical Social Worker provides services and/or establishes standards addressing the social and/or emotional factors that impact the clients’ ability to achieve the goals of the Plan of Care. Services are provided under the direction of a physician and in conjunction with other health care team members in accordance with the established policies and practices of the Agency.


Required Experience for Medical Social Worker:

  • Education and/or Experience: Master’s Degree from a school of Social Work accredited by the Council on Social Work Education. Minimum of one (1) years’ experience in a health care setting or equivalent experience. Home care experience preferred.
  • CPR certification, as applicable. 
  • Demonstrates knowledge of resources available in the community.
  • Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals. 
  • Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
  • Other: Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with good driving record.

Primary Responsibilities for Medical Social Worker:

  • Home Health Care Medical Social Worker assesses the psychosocial status of clients to determine factors that may interfere with client’s ability to achieve goals.
  • Completes assessments in a timely manner and in accordance with Agency policy.
  • Develops a written plan to provide information and direction to other health team members.
  • Documents all findings, plans, interventions, and client progress in the clinical record.
  • Communicates plans and changes to the physician and/or nursing supervisor and other Agency staff through the care plan, written progress notes, and participation in care conferences.
  • Home Health Care Medical Social Worker initiates referrals as appropriate.
  • Prepares social histories to augment existing service, or as a guide in determining or changing level of service. This may include cultural factors, financial concerns and support system.
  • Demonstrates teamwork and effective communication to accomplish client, team, and Agency goals.
  • Participates in care conferences and other team or Agency meetings. Shares areas of expertise and initiates interdisciplinary referrals.
  • Educates team members and clients/families about community resources and how to access them appropriately.
  • Home Health Care Medical Social Worker supervises the assessment, planning and implementation by the Social Service team.
  • Provides leadership to the interdisciplinary team and the Advisory Committee in identifying opportunities to improve services, and implement strategies to address unmet needs of clients.
  • Assists in the development, implementation, and evaluation of the ongoing quality improvement process.
  • Home Health Care Medical Social Worker serves on the Clinical Record Review Committee and Professional advisory Committee; attends meetings as required.
  • Work toward continual improvement of the overall organization.
  • Perform other duties as assigned.

General Working Conditions:
This position may require sitting or standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
