Position Summary:
The Hospice and Home Care Nurse is responsible for the planning, coordinating, providing and documenting care for patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the Hospice and Home Care RN develops and implements a plan of care that meets each patient’s specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines as well as the organization’s policies and procedures. The caseload for this full-time position includes a split between serving Certified home Health Care patients and Hospice patients. 



Required Experience for Hospice and Home Care Nurse:

  • Graduate of an accredited school of nursing. Baccalaureate degree preferred.
  • Current licensee as a registered nurse (RN) in the State of Michigan.
  • Minimum of two (2) years of nursing in an acute care, home care, and home hospice setting is preferred.
  • CPR certification preferred, not required.
  • Current Michigan driver’s license and/or reliable transportation.
  • Must be empathetic, nonjudgmental, tactful, responsible, adaptable, organized, and committed to the purpose and objectives of the organization.
  • Must have excellent communications skills.
  • Must have emotional and mental maturity necessary for establishing and maintaining good work relationships with patients, patient’s families, and Agency personnel.
  • Must understand the team approach and be a supportive team member.



Primary Responsibilities for Hospice and Home Care Nurse:

  • Performs physical and psychosocial assessment for all home care and hospice patients.
  • Develops a written plan of care for each patient.
  • Provides skilled nursing care.
  • Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
  • Follows up with obtaining lab results and notifying physician of any significant results.
  • Performs patient care responsibilities considering needs specific to the standard of care for patient’s age and clinical condition.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical condition.
  • Ability to adequately assess and reassess pain. 
  • Utilizes appropriate pain management techniques. 
  • Educates the patient and family regarding pain management.
  • Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassess pain and notifies the primary care physician when pain is not manageable on the current medications.
  • Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Offers ongoing support and palliative care.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality.
  • Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
  • Initiates needed referrals to other disciplines and/or community resources.
  • Supervises Hospice Aides on an every other week basis.
  • Supervises Home Health Aides every 14 days.
  • Understands Hospice philosophy and issues of death and dying.
  • Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, new problems, etc.
  • Notifies Clinical Nurse Manager of patients who need to be seen on weekends or holidays and submits updated nursing care plan to the weekend/holiday nurse.
  • Obtains needed supplies and equipment to provide care.
  • Addresses and supports cultural practices, as long as such practices do not harm others.
  • Adheres to all organization, payroll, billing and documentation policies and procedures.
  • Participates in the organization’s other mandated activities, i.e., in services, UR/PI committee, staff meetings.
